Metal braces may also sometimes dislodge because of biting, chewing, tooth movement pressure or because of the humidity in your mouth. This will require you to return to the clinic for re-attachment.

Extraction Route
Pre-Assessment
When you contact kohe™ Dental, you have the option of booking an appointment or conducting a pre-assessment with us via a photo assessment or a video consultation.
The pre-assessment and video consultation is done virtually with our dentists who will assess your current orthodontic condition, understand your main concerns and inform you of your options immediately.
Our dentists will also inform you of the cost of treatment based on your dental condition and complexity as well as the tentative treatment duration.
You will also be informed whether your orthodontic case can be a non-extraction case or not. As an non-extraction focused orthodontic centre, we prioritise no-extractions (if the circumstances allow for it).
You will also be given options for possible payment plans.
Initial Consultation (1st Visit)
If you have decided to proceed with your appointment with kohe™ Dental, your first visit will involve a 30 – 45 minutes dental consultation with our orthodontic focused dentists.
The initial consultation starts from a detailed chat with you about your concerns, goals and expected outcomes.
It then proceeds to the dental chair where your mouth and oral health are thoroughly examined.
On the same day, X-rays (OPG and Lat-Ceph) will be taken along with 3D Scans with our Shining3D intraoral scanner.
Your oral health condition will be explained to you in detail along with the treatment options that are recommended.
Should you decide on the same day that you wish to proceed with metal braces treatment, our dentist will conduct a 3D intraoral scan of your mouth and teeth.
The 3D scan forms the basis of your smile journey and helps the dentist treatment plan for your new smile.
The confirmed total amount for treatment will also be explained clearly to you.
If you wish to proceed with treatment, you will be offered two options: Upfront payment OR Instalment payment.
Should you wish to proceed with treatment, you will either pay upfront or pay the first instalment payment.
Tooth Extraction (if necessary) and Scaling and Polishing (2nd Visit)
If you have opted for extractions in the case of overcrowding or protrusive teeth, an extractions appointment will be arranged for you.
In some cases where there is a supernumerary tooth (buried tooth near the jaw), a dental specialist (Oral Maxillofacial Surgeon) might be called in to help remove the tooth before orthodontic treatment.
While we do not advocate for extraction orthodontics and believe that in most situations, teeth are important, extractions might be unavoidable in certain orthodontic circumstances where the patient does not qualify for expansion or there is simply not enough space for teeth to move properly.
Ideally, we try to retain all healthy teeth.
If you do not require tooth extractions, you will be scheduled for a pre-orthodontic scaling and polishing or Guided Biofilm Therapy session with our Oral Health Therapist.
It is important to have good oral hygiene before doing orthodontic treatment as it sets the foundation for orthodontic success.
We use an Oral Health Therapist (OHT) for all our dental cleaning work with patients as OHTs are “cleaning specialists.” They work on dental cleaning (e.g. scaling and polishing) every day and are professionals in dental cleaning.
If you do not require tooth extractions, your 2nd visit will be a braces fitting.
Metal Braces Fitting (Usually on the 2nd or 3rd Visit)
During your metal braces fitting, braces brackets will be placed on your teeth.
These brackets help to facilitate tooth movements and guide your teeth to the right positions with time.
This appointment usually takes about 45 mins to 60 mins.
An orthodontic kit with orthodontic wax will then be issued to you after your braces fitting.
Non-Extraction and Expansion Route
Expansion Therapy
If you have opted for expansion therapy, you will be offered 2 options depending on your age and stage of orthodontic development.
For children below the ages of 11 years old, a traditional palatal expander can be used to expand the upper palate to allow for more space for teeth to shift in.
For children above the age of 11 years old, and adults, MARPE is used instead.
Each of these customised orthodontic appliances allow for space creation and airway improvements.
Expander Fitting (2nd Visit)
It typically takes 2 – 3 weeks for your orthodontic appliance to return once it has been sent for manufacture.
You will be scheduled for an expander fitting once your appliance is back.
Metal Braces Fitting (6 - 12 months after expansion therapy)
Once your expansion therapy has been completed, we then move on to metal braces to help align your teeth.
During your metal braces fitting, braces brackets will be placed on your teeth.
These brackets help to facilitate tooth movements and guide your teeth to the right positions with time.
This appointment usually takes about 45 mins to 60 mins.
An orthodontic kit with orthodontic wax will then be issued to you after your braces fitting.
Affordable Metal Braces Treatment Duration
Most metal braces treatment plans range from 9 to 12 months for moderate cases, and 15 to 18 months for more complex cases.
The treatment duration will vary based on your current oral health (whether you require additional treatments before orthodontic treatment), your case complexity, and the expertise of your dentist in guiding your teeth according to the treatment plan.

What are metal braces?
Metal braces are twin brackets that help straighten your teeth.

Why are metal braces cheaper than clear aligners?
Metal braces are often cheaper and more affordable than clear aligners because of the scale they are manufactured at.
While clear aligners have to be custom fitted to your teeth, and are thus personalised based on your 3D intraoral scans or physical impressions, metal braces are produced without any personalisation (in exception of braces like Brava by Brius lingual braces).
Metal braces are not custom-fitted to your teeth. There are no high lab fees when producing metal braces. This contributes to less costs for you as a patient.
Are metal braces cheaper in Malaysia?
Metal braces can be cheaper in Malaysia. The preferential currency helps with keeping costs slightly lower.
In fact, if you are considering taking up treatment in Malaysia for braces treatment, here are some factors that you might wish to consider before taking up treatment:
- Are you willing to spend time and money to travel down to Malaysia every 4-5 weeks for braces reviews and adjustments?
- Are you willing to brave the immigration traffic for extended periods of time to head to your braces clinic?

The primary differences between metal and self-ligating braces will be as follows:
Metal Braces:

- Suitable for all ages;
- Your braces dentist has complete control over how to move your teeth and at what stage certain teeth should be moved.

How long would my metal braces treatment take?
The average duration of metal braces treatment is 9 to 12 months.
Most patients that we see fall under complex-severe and complete their braces treatment in 24 – 36 months.
Every patient has different treatment durations. The final duration is reliant on the following factors:
- How complex your orthodontic case is;
- Whether your case is an extraction or non-extraction case;
- Whether you require additional appliances before braces treatment;
- Whether you require further treatments like gum disease treatment before orthodontic treatment;
- If you are compliant to your regular braces adjustment visits.

Can I take up metal braces treatment only for either my upper or lower set of teeth?
When it comes to orthodontic treatments like braces, it is usually not likely to do treatment for a single arch.
This is because orthodontic treatment is not just about aesthetics but about functionality and bite.
Single arch treatment is usually done as a “finishing” treatment post-metal braces or clear aligners treatment. It is done to refine the appearance or correct final adjustments after your primary orthodontic treatment is done.
If you are intending to take up metal braces treatment, it is not likely that you will be able to do single arch treatment.
Which braces can move teeth the fastest?
If you are looking to expedite treatment or would like the fastest and safest braces treatment available, Brava by Brius braces can shift teeth up to 45% faster than traditional metal braces.
Self-ligating braces are also able to shift teeth up to 35% faster than metal braces.
Both options allow for safe and efficient teeth straightening.
At what age are braces most effective?
Early orthodontic treatment can help to reduce braces treatment duration significantly.
We usually recommend an early orthodontic assessment from as young as 5 years of age to check in on bite, teeth alignment, airway and breathing, and correct poor oral health habits.
The best age to seek orthodontic treatment like braces would be as soon as most permanent teeth have emerged and the jaw is still growing, making teeth easier to move. This is usually between the ages of 10 to 14 years old.
Is my child too young for braces?
An early orthodontic assessment is usually recommended from the age of 5 onwards to determine whether your child is suitable for braces.
The good thing is that the earlier you identify orthodontic problems, the easier and more affordable it is to provide a solution for them.
In some cases, this also means preserving natural teeth instead of extracting them during orthodontic treatment.
Can braces fix all orthodontic conditions (e.g. upright a molar)?
Braces can help fix most orthodontic conditions like overjet, a protrusive lower jaw, misalignment of upper and lower teeth, crowding, correct midline misalignments, etc.
Gold chaining can also be used in combination with braces to upright a second molar instead of removing it.
However, just like any other orthodontic treatments, there are some limitations.
Braces will not be able to correct a narrow palate. Additional appliances like MARPE (for aged 11 and above) or RPE (for aged below 11) will have to be employed for the expansion of the palate.
Braces also cannot correct jaw asymmetry as this is a skeletal issue. For cases where there is jaw asymmetry, you can choose to either do a braces camouflage or undergo orthognathic surgery to correct it.
What can influence and affect my braces treatment result?
If you are intending to take up braces treatment, the following can affect the outcomes of your braces treatment:
- Poor oral hygiene: Infrequent or negligent brushing and flossing of your teeth during orthodontic treatment can increase the risk of tooth decay and gum disease.
- Refusal to attend regular oral maintenance dental cleanings during orthodontic treatment: It is important to maintain healthy teeth and gums during orthodontic treatment to prevent gum disease and dental decay.
- Failing to return on time for braces review and adjustments appointments: Doing so can result in your treatment duration extending. Braces brackets do not automatically shift teeth and require frequent adjustments to achieve your desired position. Not returning for monthly reviews can cause inferior results and increased treatment time.
- Failing to comply with clinician-directed use of braces accessories/rubber bands: The quality of the treatment outcome can be severely affected when appliances are not used when indicated by your braces dentist.
What are the limitations and risks of braces treatment?
Just like any dental or orthodontic treatment, braces has certain limitations and risks:
Limitations:
- Unable to expand narrow arch or palate and will require additional appliances for it;
- Unable to correct jaw asymmetry;
- Might have a longer treatment duration in comparison to alternative teeth straightening treatments like clear aligners or Brava by Brius lingual braces.
Potential Risks:
- Poor oral hygiene can cause tooth decay or white lesions forming during orthodontic treatment;

Do I need to remove my wisdom tooth/teeth before my metal braces treatment?
It is not always necessary to remove your wisdom teeth or teeth before orthodontic treatment.
In some cases, there is no necessity to remove teeth or wisdom teeth for braces treatment.
In fact, it is our clinic philosophy not to extract teeth if possible.
There are circumstances where your wisdom teeth or teeth might need to be removed before proceeding with orthodontic treatment:
- Wisdom tooth is carious;
- Wisdom tooth is infected or inflamed;
- Wisdom tooth is impacted and might affect the movement of your tooth roots during orthodontic treatment;
- Your teeth might be buried below another tooth which prevents proper orthodontic movement;
- You have severely overcrowded or protrusive teeth and require additional space to be created for your teeth to retract inwards.
